The Reaching for Excellence initiative released findings from an unprecedented region-wide forum engaging more than 1,700 Western New Yorkers in a series of conversations about the future of health care in the region. Emerging from these conversations were five community priorities for health care, including the need for care that is delivered with compassion, more efficient and coordinated delivery of care, and patient empowerment through information. To guide the region’s health system in quality improvement on community priorities, Reaching for Excellence has released a system of performance measurement tools, including data-driven indicators. In addition to monitoring regional progress in priority areas, the effort will continue to foster regional dialogue on health care improvement through such programs as a Speaker Series.
